**All Medical / Release Forms for the Junior and Senior School must be complete by the day of camp registration. A copy of an up to date physical OR completed Medical / Release Form with a physician signature are required. Registration will be quicker if the forms are completed and sent to Jeb Burch prior to the day of registration**
